Lermoyez's Syndrome A Case Report

I．緒言

　Lermoyez症候群は難聴，めまい，耳鳴をきたすという点でMénière病と類似した疾患であるが，症状の出現順序においてつぎのような特徴を有することからMénière病とは区別されている。すなわちMénière病ではめまいが他の症状に先行するか同時に起こるのに対して，本症では「難聴がめまいに先立って始まり次第に進行するが，めまい発作が起こると難聴は突然に回復する」という特徴を有する。本症は比較的まれな疾患であり，われわれが調べた限りではこれまでに確定診断されたのは30例ほどにすぎない。

　今回われわれは典型的な本症の1例を経験したので，若干の文献的考察を加えて報告する。

　A 65-year-old man presented with a two-month history of vertigo and hearing loss. During hospitalization, his hearing had gradually worsend until vertigenous attack was occurred. After the attack, his hearing prominently recovered to the level before the attack. This history suggested typical Lermoyez's syndrome. Audiological and vestibular findings proved the presence of endolymphatic hydrops which is typically seen in Ménière's disease. The difference between Ménière's disease and Lermoyez's syndrome was discussed.
