口蓋扁桃にみられたangioimmunoblastic lymphadenopathy with dysproteinemia（AILD）症例

I．はじめに

　AILD（angioimmunoblastic lymphadenopathywith dysproteinemia）はFrizzera^1）により1974年に初めて報告された疾患概念である。全身のリンパ系臓器の腫脹を主徴としさまざまな臨床症状を呈し，他のリンパ系腫瘍，とくにHodgkin病との異同について議論があるが，現在のところ一応独立した疾患概念としてとらえられている。頭頸部リンパ節腫脹を呈する症例が多いにもかかわらず，上気道リンパ組織，とくに扁桃に関する報告はHug^2）の1例報告以外にはない。

　今回われわれは口蓋扁桃にAILD病変を認めた41歳女性症例を経験したので報告した。

　Angioimmunoblastic lymphadenopathy with dysproteinemia (AILD) in a 41-year-old house wife with recurrent tonsillitis was reported. She presented with fever, night sweat, weight loss and cervical lymphnode swelling of few months' duration, and these were characteristic features of AILD.

　Histological examinations of the cervical lymph-nodes and the palatine tonsil showed proliferation of arborizing small vessels, prominent immunoblastic proliferation and destruction of follicular architectures.
