先天性頸耳瘻の1症例

A Case of First Branchial Cleft Anomaly

I．はじめに

　第1鰓溝はヒト胎生2週〜8週における胚子の外形を特徴あるものにしており，将来外耳道・鼓膜の外側上皮へと変化する。その発生過程において異常が発生すると，外耳周囲の先天性奇形が出現する。第1鰓溝の先天性奇形は比較的稀なもので，第2鰓溝性のそれと比して6：1の割合で発生するとも報告^1）されている。著者らは第1鰓溝の先天異常である先天性頸耳瘻の症例を経験したので，文献的考察を加えて報告する。

　A 1-year-and-9-months-old boy developed reccurent purulent discharge from the car and a fistula.

　The fistulous tract located in the anterior triangle of the neck, extending upwards under the facial nerve and opened into external auditory canal.

　The fistula was removed and the facial nerve was preserved. Microscopic examination revealed a lining of squamous epithelium with adnexal structures and cartilage.
