止血剤によりアナフィラキシーショックをおこした軟口蓋pleomorphic adenomaの1例

A Case of Pleomorphic Adenoma, and Anaphylactic Shock Induced by Hemostatics

I．はじめに

　薬物アレルギーの症状としては，種々のものがあるが，そのうち最も重篤なものは，アナフィラキシーによるものであるといわれる。アナフィラキシーをおこす薬剤としては，麻酔剤，抗生物質をはじめ多くの薬剤にその可能性があるが，止血剤によるものは比較的稀とされている。今回，われわれは，止血剤によりアナフィラキシーショックをおこした軟口蓋Pleomorphic adenomaの1例を経験したので報告する。

　A 33-year-old man complained of a swelling of the soft palate, and CT-scan and sialography revealed that the parapharyngeal space was filled with a tumor. During the operation under generalanesthesia, the patient was given an injection of hemostatics such as Adona, Kaytwo and Transamin. About 5 minutes later he developed erythema, peripheral edema and severe hypotension. Emergency treatment was immediately performed and the patient recovered after about 3 hours. Four months later, the tumor was successfully removed under general anesthesia without using of hemostatics, and the tumor was found as pleomorphic adenoma.
