IgM産生口蓋扁桃形質細胞腫の1症例

IgM Producing Plasmacytoma of the Tonsil

　I．緒言

　耳鼻咽喉科領域における腫瘍のうち，形質細胞腫は比較的稀なものである。筆者らは最近，左口蓋扁桃より発生した形質細胞腫痒例を経験すると共に，腫瘍より産生された免疫グロブリンについて詳細に検討し，細胞内にはIgMとIgG（ともにλ型）2種のIgが産生されているのに対し，血中にはそのうちIgM（λ）のみを分泌している特異な形質細胞腫であることを明らかにした。形質細胞腫産生免疫グロブリンについて詳細に検索されたものは現在まで数例を数えるのみで少なく，また腫瘍組織の産生Igを直接，酵素抗体法を用いて検出したとの報告はない。本例で得られた知見と共に文献的考察を加えここに報告する。

　A 74-year-old male with plasmacytoma of the left tonsil is reported. Monochronal IgM was found in the patient's serum, and IgM (λ) was also detected by immunoelectrophoretic method. It was of particular interest that the monochronal IgM and IgG were observed in the tumor tissue by enzymelabeled antibody method. It is assumed that the change of immunoglobulin from IgM to IgG was occurred when the tumor started to grow in the left tonsil.
