多発性脳神経障害を伴うHunt症候群の2例

TWO CASES OF HUNT SYNDROME ACCOMPANIED WITH MULTIPLE CRANIAL NERVE SYMPTOMS

　I．はじめに

　Hunt症候群は，膝状神経節の帯状疱疹ウイルス感染によると考えられる疾患で，外耳道および耳介とその周囲に帯状疱疹を認め，顔面神経麻痺ときに内耳神経症状を伴う。しかし，それらの神経以外の脳神経をも含む，いわゆる多発性脳神経症状を伴うHunt症候群の報告は少ない。今回われわれは多発性脳神経症状を伴つたHunt症候群の2例を経験したので，文献的考察を加えて報告する。

　A man, aged 81, developed of aphonia and dysphagia with herpetic eruptions in the right auricle, and the careful examination revealed the presence of disturbances in the right 9th and bilateral 10th cranial nerves without mucosal symptoms of the mouth. pharynx and larynx. He completely recovered after two weeks of treatment.

　The second case, a man aged 52, complained of right facial palsy and difficulty swallowing proceded by high fever and earache of the right side.The examination showed disturbances of the right 5th, 7th 8th, 9th, 10th and 12th cranial nerves. After three weeks of treatment, his symptoms were improved except for moderate disturbances of the facial and cochlear nerves.

　The prognosis of the impaired cranial nerves except for the facial and cochlear nerves was generally good in our cases and in the others which had previously been reported in Japan.
