放射線治療後再発した舌癌の進展様式—臨床病理学的検討

RECURRENCE OF LINGUAL CARCINOMA FOLLOWING RADICAL IRRADIATION TREATMENT AND THE MODE OF ITS PROGRESS

　舌癌の治療としては機能保存の目的から頸部転移のない場合，第一に放射線治療が選択される。われわれのグループにて昭和41年から48年末までの8年間に放射線単独治療の対象となつた88例のうち16例は腫瘍が消失せず，また残る72例は一時腫瘍の消失がみられたが，そのうち22例（30.5％）が再発をきたしている。つまり放射線治療のみでコントロールできなかつた症例は合計38例（43.2％）と意外に多かつた。

　われわれは先の報告^11）において腫瘍の進展度，組織分化度，放射線線源および年齢と再発との関係を検討したが，いずれの要因とも相関を認めなかつた。また再発群は放射線による初回治療により腫瘍が消失しなかつた群に比して予後は悪く，特に腫瘍死に到る症例が多かつた。

　For the treatment of lingual cancer, irradiation therapy appear to be the treatment of choice. But, more often than not, this treatment alone is insufficient in overcoming the affection.

　In order to investigate the cause and the manner by which recurrences may recur, detailed examinations were undertaken by clinical and laboratory methods.

　In cases of recurrences that occurred within a year following the treatment the size of the growth differed from those that showed a later recurrence. With early recurrences the growth reached the original size while late recurrences were much smaller. The large sized recurrences required a larger piece of biopsy while the smaller ones sufficed with a smaller piece.
