甲状腺髄様癌の1症例

MEDULLARY CARCINOMA OF THE THYROID GLAND

　I．緒言

　甲状腺髄様癌は，血中Ca濃度を低下させるホルモンであるcalcitoninを分泌する甲状腺傍濾胞細胞由来の癌腫であり，褐色細胞腫などの合併や家族性発生がみられるなど興味深い性質を有する。また，最近ではradioimmunoassay法が開発され血中calcitonin定量により本疾患の早期発見が可能になつてぎた。

　われわれは，甲状腺髄様癌症例を経験したので報告し，文献的考察を加えた。

　A case of medullary carcinoma of the thyroid gland without pheochromocytoma in a woman, aged 46, is reported. The family history was negative. The chief complaints were a mass in the neck and hoarseness of voice.

　The examination showed the left vocal cord to be fixed in its mid-position. Fifteen months after the Patient's first visit to the clinic, the tumor was diagnosed as a medullary carcinoma of the thyroid gland.

　Subtotal thyroidectomy with left radical neck dissection was performed.

　The carcinoma invading the mediatinum was treated by means of Co⁶⁰ irradiation of 4,800 rads following the operation.
