難治性慢性外耳道炎—特にその臨床像と真菌アレルギーの可能性をめぐつて

　Ⅰ．緒言

　鼓室成形術が行なわれたにもかかわらず，なお耳漏の続く患者がある。なかでも不思議なのは，術中活動性病変が僅かであつたにもかかわらず，術後耳内の湿潤状態がいつまでも続き，治療を続けないと，いつの間にか外耳道に粘膿性の分泌物がたまるという，はなはだ理解に苦しむ症例の存在したことである。

　本論文は，このような耳漏症例を集めて検討を加えたものである。その結果，耳漏の原因が，慢性外耳道炎にあることを知つた。これは抗真菌剤による治療とアレルギー減感作療法で乾燥化させることができた。そこでまずこのような耳漏症例の概略を報告し，ついで本症の臨床的特徴や成因などについて箸者の意見を述べてみた。大方のご批判を得たくここに報告する次第である。

　Five patients with chronic external otitis due to fungus infection are reported. This disease though it may not be highly infectious has a tendensy to be confused with chronic otitis media. In the diagnosis, it should be confirmed that the aural discharge is not the product of the middle ear, and with microscopic proof of the presence of the fungi. All five patients showed positive response to delayed type IV hypersensitivity skin test with candida extract; the aural discharges decreased to produce dry ears.
