開鼻声を主訴とした小児重症筋無力症の1症例

HYPERNASALITY AS THE CHIEF SYMPTOM IN A CASE OF MYASTHENIA GRAVIS

　I．はじめに

　われわれが耳鼻咽喉科で扱う疾患の中には，全身疾患の一部分症としての症状を呈してくるものが数多くある。しかし日常の診療では，とかく局所所見にとらわれるあまり，時に，その背景にある全身疾患との関連を忘れることもある。特に全身の症状がほとんどなく，耳鼻咽喉科的局所症状が前面に出現した場合は，その傾向が強い。

　今回われわれは，開鼻声を訴えて来院した小児の重症筋無力症を経験したのでここに報告しその耳鼻咽喉科的症状と診断法について述べてみた。この症例は全身症状に乏しく，顔面鼻咽腔に無力症状を示す，いわゆる球型といわれるものであり，診断に苦渋したが，Tensilon testにより初めて確診を得た次第である。

　A case of myasthenia gravis in a girl, aged 11, with hypernasality of voice as the chief complaint is reported. The diagnosis of myasthenia gravis is confirmed by tensilon test and electro-muscular fatigability test. Due to the presence of such symptoms as hypernasality or dysphagia the patients with myasthenia gravis have tendencies to seek the service of ENT, for which the department should be prepared. When bilateral paralysis of the soft palate occur this disease should be on the lookout.
