失明を合併した前頭洞ピオツェーレ

AMAUROSIS DUE TO FRONTAL PYOCELE

　I．はじめに

　抗生物質を使用するようになつて炎症性疾患の合併症が減少した。鼻副鼻腔疾患においても急性炎症の合併症は稀になつたが，一方において術後性頬部嚢腫，ムコツェーレ，ピオツェーレなどは日常の臨床において比較的よくみられる。これらの症例も手馴れた治療方針で時期を失することなく治癒している。前頭洞のピオツェーレにしてもこのような事情から症例報告になることはほとんどなく，もはや古典的疾患として注意をひくことが少ない、しかし時として普段われわれが見馴れた疾患が古典的成書にしか詳しい記載のない合併症をきたすことがある。この症例報告はその1例であつて，前頭洞ピオツェーレから軽度の頭蓋内合併症と眼窩内骨膜下膿瘍を生じ，ついで永続的な失明を残したものである。診断と失明にいたる経過に検討の予地があるため報告したい。

　A man, aged 55, was found to be affected with a pyocele of the frontal sinus that caused an orbital complication. The patient complained of exophthalmos, orbital pain and a severe headache at the left parietal region. An ophthalmologist made an incision in the upper eyelid for an attempt at drainage. Three weeks after the onset of the disease a complete amaurosis was noted.

　The patient was finally treated surgically by a complete frontal sinusectomy with uneventful recovery. However, the lost sight showed no change within the 5 months after the opera-tion.
