副腎皮質ホルモン使用者に対する扁桃摘出術について

TONSILLECTOMIES AMONG PATIENTS UNDER ADRENAL CORTICAL HORMONE TREATMENTS

　I．はじめに

　副腎皮質ホルモン（ステロイドホルモン）を投与されていた患者に手術のよりなストレスを加えることは急性副腎不全を招来する可能性があり，危険であることはよく知られている事実である。1952年Fraserら^1）がステロイド使用のリウマチ性関節炎の患者に手術を行ない，死亡したことを報告して以来，1950年代にはかなりの死亡例が報告されている。扁桃摘出術はそれ自体，シヨックなどを起こす可能性があり，副腎皮質ホルモンが使用されていればさらにその危険性は増大するのではないかと懸念される，しかしながら腎炎，リウマチ性疾患などで副腎皮質ホルモンが使用され，原病巣としての扁桃を除去しなければならない場合も少ないとはいえない。わたくしどもは最近1年半にこのような症例35例に扁桃摘出術をしたので若干の考察を加えて経験を述べてみたい。

　The result of tonsillectomies performed on patients who were treated under the administration of adrenal cortical hormone for rheumatic and other related affections. The dose of this medication ranged from 60 mg to 1,843.5 mg which was given in 2 days the shortest to 188 days the longest, with the average of 40.9 clays in duration.

　The operation was performed both, under local and general anesthesia. No adverse effect was seen because of this specific medication given preoperatively.
