耳鼻咽喉科領域に於けるKimopsin（Alpha-Chymotrypsin）の臨床的応用

　I．緒論

　耳鼻咽喉科領域に於ける炎症性疾患は単に特定の細菌感染によるだけの病因でなく，アレルギー性の病変を含めて，外界の物理的，化学的な刺激に対する生体の反応形式としての炎症も又少くない。これら炎症性疾患の治療にあたつて，ズ剤，抗生剤をいかに用いても慢性炎症の殆んどが適確な治療効果を挙げ得ない現況にある事は，副鼻腔の炎症や或る種の慢性中耳炎等にこれを見る事が出来るし，最初から細菌感染と言うより生体の過剰な防禦反応の現われである如き炎症は，急性鼻咽頭炎や耳管中耳カタルの或るものにも見い出し得る。要するに炎症に対する治療法としては，特に耳鼻科領域に於いてはズ剤，抗生剤の対細菌作戦のみでは不適当な疾患が比較的多いと考えられる。かかる難治の疾患に対して，副腎皮質ホルモンの発現とその応用は極めて効果的であつたが，それとても一面には色々な副作用を伴う為充分且つ安全な治療が難かしい情況である。特に此の領域では比較的軽い（全身的に影響を及ぼすとか，日常生活に差支える程度とかの疾患ではなしに）しかも頑固な慢性炎症が多い所から或る意味で局所の病理解剖学的な変化を改善する如き抗炎症性薬物が副作用なしに充分使用出来る事は吾人の久しく望む所であつた。

　Kimopsin（α-Chymotrypsin）はかかる意味で吾々の希求した薬物の一つである事が外国文献に数多い治験例と共に記載されておるが，今般その試用の機会を与えられ，事実それ程の効果があるかどうか，又耳科領域として如何なる疾患に用い得るか，或は如何なる使用法が適当であるか等を験する事が出来た。ここに報告して御検討御批判を仰ぐものである。以下暫らく内外の文敵よりKimopsinの性質を概記する。

　A review of some articles on Kimopsin (α-Chymotrypsin) is held especially on the points of its biochemic and pharmacologic natures, and 74 cases of oto-rhino-laryngological diseases which have been treated generally or topically by Kimopsin solution is described.

　Concentration of Kimopsin solution which is topically used is similar to that for injection, and mixed solutions of Kimopsin with some antibiotics, antiallergic drugs, antihistamines and steroids are used in tympanic cavity, nasal cavity, sinuses and upper respiratory tract.

　Topical use of Kimopsin solution shows mu cheffect on the aural discharge which proves no bactcrium by kultre.

　The effects of displacement method for sinusitis and of aerosol treatment for acute or chronic inflammation of upper respiratory area by Kimopsin solution are excellent and other possibility of indication of Kimopsin in oto-laryngological section are described.
