Operation Audiometryについて

　I．緒言

　最近我国に於ける鼓室成形術（Tympanoplastic operation）の急速なる発達と共に伝音系難聴耳に対する伝音機構障害部位に関するより精密なる診断が要求される様になつて来ている。以上の目的を達成する為に，外国では主にSchall-Sonde（sound probe）を利用する所謂Sonden-prüfungの発達を来たし，又，我国に於いても角田氏の液導差による鐙骨固着度測定法等の発達を見た。然し，以上の精密診断の要求を満足する為には尚Sonden-Prüfungだけでは不充分である様に思われ，例えば正円窓機能診断に当つては更にBaffle Effectを利用する各種の検査方法も必要となつて来る。従つて伝音障害に関する精密診断法に対してもSondenprüfungと名づける事は妥当ではなく，1957，Lundborgは"Transmission Testing"と名づけている。我々は手術施行に必要なる聴力検査法と云う意味からこれにoperation audiometryと名づけて研究を行つて来た。斯くして，50例の慢性中耳炎に対して手術前は勿論手術中もOp. Audiometryを施行し，且つ全例に鼓室成形術を施行し手術所見と検査成績とを比較検討し吟味を行つた。尚，Op. Audiometryは各種伝音障害の状態に関する診断を必要とするのであるが，本論文に於いては特にSonden Prüfungを利用し聴小骨に関する定性的診断方法並びに正円窓機能検査方法の一部に就いて報告する。

　Operation audiometry is the term applied to a type of hearing test which is a requisite in tyrr, panoplasty. Employing Schall sound definitive diagnosis of conductive disturbances is established. By Bennet's disrupted ossicular chain test the majority of cases showed atypical types of disturbances which were quite in agreement with what Lundborg calls as acoustically "short circuit": these cases by operative observations seem to agree with Gehor's so-called cholesteatoma. The disturbances in the middle ear ossicular chain by definitive diagnosis may divided into those that are (a) nomal, (b) short circuit and (c) disrupted chain. By employment of the sound functional test of the round window also may be conducted.
