YAMAKAWA and NAITO maintain that Portmann's operation for treatment of Meniere's Disease is justifiable in as much as the latter, today, is recognized as symptom complex resulting from endolymphatic hydrops. This operation which is an attempt for establishment of endolymph drainage is performed extra-dural; the desired drainage may be obstructed by formation of granulation tissues only to cause recurrence of symptoms. In order to avoid complications of this nature authors modify the procedure by making, after opening the saccus endolymphaticus by fenestration, a L-shaped incision in the dura and overturning the triangular flap which establishes thereby an unobstructed circulation of the endolymph and the cerebro-spinal fluid. Of the 12 cases of Meniere's disease who were operated upon by such modified method 8 are completely cured ; four are improved.
