メニエール氏病に對するホルトマン手術の變法—山川・内藤式手術

　メニエール氏病の内耳病變が非炎症性内リンパ水腫である事は剖検例によつて明かにされた事實であるが，更にその原因が何であるかは今日殆んど判つていない。從つてメ病の原因を除去すると云うが如き根本的療法を望む事は現状に於ては無理である。

　そこで内リンパ水腫を除く爲に内リンパ嚢を切開して内リンパを内リンパ腔外に導き之を吸收せしめる方法が考えられる。ポルトマン手術が即ち之である。然しポルトマン法^4）6）は周知の如くペニシリンがなかつた時代に考案されたもので硬腦膜を切開する危險を避け内リンパ嚢外壁を切開したものであり，術腔は骨内である爲吸收が不充分であると考へられるし，又術腔は何れ肉芽瘢痕で充たされるのであろうから，折角切開を行つた内リンパ嚢の部分もその爲に鎖されてしまう心配がある。即ち術後半年なり一年なりして見られる再發はこの爲に起るのではなかろうかと考える。そこで私達はこの不安を除く爲に硬腦膜内側に於て内リンパ嚢内壁に窓をあけて（單なる切開ではなく）内リンパが廣いリコール腔へ常時流出可能となる如き方法を考案した。（第1圖・第6圖）

YAMAKAWA and NAITO maintain that Portmann's operation for treatment of Meniere's Disease is justifiable in as much as the latter, today, is recognized as symptom complex resulting from endolymphatic hydrops. This operation which is an attempt for establishment of endolymph drainage is performed extra-dural; the desired drainage may be obstructed by formation of granulation tissues only to cause recurrence of symptoms. In order to avoid complications of this nature authors modify the procedure by making, after opening the saccus endolymphaticus by fenestration, a L-shaped incision in the dura and overturning the triangular flap which establishes thereby an unobstructed circulation of the endolymph and the cerebro-spinal fluid. Of the 12 cases of Meniere's disease who were operated upon by such modified method 8 are completely cured ; four are improved.
