—佐藤教授開講二拾週年記念論文—經鼻手術を行ひたる腦下垂體部腫瘍症例

　腦下垂體部に現れる腫瘍は該部が腦下垂體の存在部位である關係上，往時は腦下垂體腫瘍として取扱われたのであるが，之が腦下垂體より發生する腫瘍だけでなく，從つて腦下垂體障碍による内分泌異常症状を示さない事も多く，臨床的には視神經障碍がより有意義で主徴候となり易いので腦下垂體部腫瘍，或いは視神經交叉症状群（Chiasma syndrom）を呈する腫瘍として近時格段の進歩を遂げた腦外科の對照となつている．そしてこの部腫瘍に對する治療としては適切に行われた手術的療法が最も有効の樣であるが，是の手術的療法の沿革を見ると本世紀初頭より開頭術の黎明と共に行われたが殆んど成功せず，之が經鼻手術特にHirsch（1910）のseptale Methodeにより良成績を得るに至つてCushingをして一旦は腦下垂體腫瘍のみは經鼻法が良いと迄言わしめたが，その後の開頭術の進歩は遂に經鼻法の成績を凌駕するに至って今日に至つている．而らば經鼻法は全く過去の舊法としてその適應を失い，存在の意義を沒したのであろうか．この點に於て最近の寡小なる例而も成功例ではないが，Hirschの方法を私が改良した方法で行つた經驗より些か考察してみたい．因みに我が邦で最初にseptale Methodeを行つたのは大正3年（1914）和田氏（大日耳鼻22卷下）の樣であるが，その後の本法の追試は殆んど數ふるに足りない樣である．

　By modified Hirsch's method the author performed pernasal operations on tumors of sella turtica. A curettage of ethmoid cells is done and then through the nasal septum the sphenoid sinus is entered and finally sella is reached.

　Case 1: A 35 years old female. The vision became worse after the removal of what proved to be a benigh tumor.

　Case 2: A 42 year old female who complained of an impaired vision was diagnosed as craniopharyngeal tumor. The operation was successful but the vision remained unimproved. The author maintains that in certain cases of this tumor, pernasal method is preferable to percranial route in that former would serve a larger operative field and prevent a possible infection by the use of chemotherapy.
