くも膜下出血にて発症したM1-2部解離性中大脳動脈瘤の1例

Dissecting Aneurysm of the Middle Cerebral Artery (M1-2 portion) with Subarachnoid Hemorrhage: A Case Report

I．はじめに

　近年，解離性中大脳動脈瘤の報告が増加し，治療についても保存的治療から積極的な外科治療まで様々な試みがなされるようになった．今回，右頭頂後頭葉皮質下出血で発症し，当初の脳血管撮影では異常を認めなかったものの，その7日後にくも膜下出血を生じ，右解離性中大脳動脈瘤の出現が判明した1例を経験したので文献的考察を加え報告する．

We report a SAH case of a ruptured dissecting aneurysm of the middle cerebral artery following parieto-occipital subcortical hemorrhage. A 68-year-old woman was admitted to our hospital, complaining of headache. On admission she was alert with left homonymous hemianopsia. A CT scan disclosed subcortic-al hemorrhage in the right parieto-occipital lobe. An angiogram revealed no abnormal vessels. Seven days after admission, she suddenly lapsed into unconsciousness with left hemiparesis.
