中頭蓋窩硬膜より発生したintracranial schwannomaの稀な1例

A Dura-based Intracranial Schwannoma in the Temporal Fossa : A Case Report

Ⅰ．はじめに

　Schwannomaは原発性脳腫瘍の10.8％を占め，その多くは第8脳神経のvestibular divisionの末梢部より発生する^10,14）．しかしながら時に，脳脊髄神経と関連なくschwannomaが発生することが知られており，現在まで60例を超える報告がなされている．発生部位別に分類，検討されているが，発生由来については未だに不明点が多い^10）．

　今回われわれは，脳脊髄神経に由来せず中頭蓋窩硬膜に発生し，側脳室側角部に進展したschwannomaの稀な1例を経験したので報告する．

We report a rare case of a dura-based intracranial schwannoma in the temporal fossa.

A 77-year-old man admitted to our hospital with a 5 months history of mild recent memory disturbance. Magnetic resonance imaging (MRD revealed a slight enhanced solid mass with a cystic mass in the right temporal lobe. Preop-erative diagnosis was glioma and then total resection was performed by the transsylvian approach. Dense adhesion of the tumor to Jura matter of the middle cranial fossa was confirmed during surgery.
