I．はじめに

　TSH産生下垂体腺腫は，下垂体腺腫のうちで非常に稀であり内分泌学的にも興味のある疾患である．われわれはこの腫瘍が近傍組織に進展及び浸潤を示し，しかも線維性で一部に石灰化を有していたために摘出に難渋した症例を経験した．本自験例を報告するとともに，このhard pituitary adenomaに関して文献的考察を加えた．

TSH-secreting pituitary adenoma with calcification and proliferation of the collagen fibers was presented. A 42-year-old man had shown general fatigue and thy- roid hypertrophy caused by hyperthyroidism for 3 years. CT and MRI revealed pituitary adenoma with calcifica-tion extending into the cavernous sinus and sphenoid sinus. The patient was operated on using the transsphe-noidal route twice, but the tumor was not able to be re-moved totally, partly due to the hardness of the tumor. The tumor in- and around the left cavernous sinus as well as the hardest part of the tumor itself due to the calcification could not be removed. Histopathological examination revealed chromophobe adenoma with pro-liferation of the collagen fibers. Immunohistological and electronmicroscopic examination demonstrated TSH-secreting adenoma. Postoperatively, thyroid function improved and the patient's symptoms due to hyperthy-roidism disappeared.
