肺，眼サルコイドーシスを併発したリンパ球性下垂体炎の1症例

A Case of Lymphocytic Adenohypophysitis Associated with Sarcoidosis of the Lung and Eye

I．はじめに

　妊娠や分娩を契機に，視力障害や下垂体前葉機能不全で発症する下垂体病変の1つにリンパ球性下垂体炎がある．その発生機序として自己免疫系の関与も示唆されているが，確定診断後の臨床経過の推移をとらえた報告は少ない．今回，われわれはリンパ球性下垂体炎後の下垂体前葉機能不全に対し内分泌補充療法を継続していたところ，6カ月後に肺および眼サルコイドーシスを併発した極めて稀な1例を経験した．両疾患とも現在のところ明らかな病因は不明であるが，全身の免疫異常の観点から非常に興味がある症例と思われたので若干の文献的考察を加えて報告する．

Abstract

Lymphocytic adenohypophysitis is considered to be an inflammatory disease of the adenohypophysis that is commonly present with visual disturbance and hypopi-tuitarism. Its etiology remains unclear but it is often re-lated to an autoimmune disorder involving other organs, such as the thyroid, parathyroid, or adrenal glands. We encountered a rare case of lymphocytic ade-nohypophysitis associated with sarcoidosis of the lung and eye during the follow-up period.
