眼動脈が前大脳動脈A1から起始していた傍前床突起部内頚動脈瘤の1例

Anomalous Origin of the Ophthalmic Artery from the Anterior Cerebral Artery Associated with the Paraclinoid Internal Carotid Artery Aneurysm

Ⅰ．はじめに

　眼動脈起始異常の中では中硬膜動脈（middle meningeal artery：MMA）起始部や海綿病脈洞部内頚動脈（internal carotid artery：ICA）からの分岐が多く報告されているが，前大脳動脈（anterior cerebral artery：ACA）A1より眼動脈が起始する症例は極めて稀である．胎生期でのprimitive ventral ophthalmic artery（PVOA）遺残がACA A1からの眼動脈起始につながったものと推測される．本症例は，傍前床突起部内頚動脈瘤を精査中に眼動脈がA1に起始する破格が判明した．今回われわれは，眼動脈起始異常に合併した傍前床突起部内頚動脈瘤の1例を経験したので，文献的考察を踏まえて報告する．

　The ophthalmic artery usually arises from the supraclinoid portion of the internal carotid artery. Here, we present an extremely rare case of abnormal origin of the ophthalmic artery from the anterior cerebral artery associated with the paraclinoid internal carotid artery aneurysm. As the embryology of the ophthalmic artery is complex, this case provides additional insight into the variation of the ophthalmic artery.
