閉塞性椎骨脳底動脈病変に対する外科的治療

Revascularization for Vertebro-basilar Occlusive Disease

I．はじめに

　椎骨脳底動脈系の閉塞性疾患に対する外科的治療は，鎖骨下動脈や椎骨動脈起始部病変を除き歴史も浅く，手術用顕微鏡が導入されて以降，最近になって脚光を浴びてきた．現在まで頭蓋内外ともに，これらの閉塞性椎骨脳底動脈病変に対してさまざまな血行再建術が報告されている^{1-4,6,8-10）}．しかし，個々の症例においていかなる手術手技を選択すべきかについては，いまだ十分な検討はない．われわれは，以下に述べる血行再建術を経験しているが，これを基に各々の手術手技の利点および欠点を明らかにし，手術法の選択について検討を行った．

Various surgical techniques have been reported for vascular reconstruction in cases of vertebro-basilar occlusive disease, but sufficient study has not been made on the question of which technique should be ap-plied in various pathological conditions. Based on our experience, we have examined the advantages and dis-advantages of these techniques.

In 34 patients with clinically and angiographically di-agnosed vertebro-basilar insufficiency, the reconstruc-tive vascular surgery to the posterior circulation was performed.
