非機能性下垂体腺腫における高プロラクチン血症—85例の検討

Hyperprolactinemia in the Patients with Non-Functioning Adenoma: Analysis of 85 patients treated with transsphenoidal operation

I．はじめに

　下垂体腺腫の中にはプロラクチン（PRL），成長ホルモン（GH），ACTHなどの下垂体ホルモンを分泌し，さまざまな臨床症状を示すものもあれば，α—subunit分泌腺腫も含めて臨床症状を発現しない非機能性下垂体腺腫も存在する．そのような非機能性下垂体腺腫の鞍上部伸展により，下垂体茎または視床下部が圧迫をうけて高PRL血症をきたすことはよく知ら^7,11）．われわれは経蝶形洞手術を施行した85例の非機能性下垂体腺腫について，それらの患者の病態を解析し，特に血清PRL濃度に注目して検討した．

Today, many gynecologists consider that the first choice of the treatment of prolactinomas is bromocrip-tine therapy. Because bromocriptine not only de-creases the levels of serum prolactin but also reduces the tumor size. On the other hand, the patients with non-functioning adenoma sometimes show hyperprolac-tinemia, probably because PIF (prolactin inhibiting factor) cannot reach the normal prolactin-producing cells of the adenohypophysis. Therefore non-function-ing adenoma with elevated serum prolactin levels should be distinguished from prolactinoma.
