I．はじめに

　巨大脳動脈瘤は，その発症，経過および治療において他の脳動脈瘤と異なった特徴を示し，特にその手術においては非常な困難を感ずることが少なくない，その発生頻度は脳動脈瘤症例の5％程度と考えられ^2），多発性脳動脈瘤の発生率が10-20％とされることより^12），多発性巨大脳動脈瘤は全脳動脈瘤症例の0.5-1.0％を占めると推定される．Foxら^12）の成人の巨大脳動脈瘤のシリーズ748例では多発例は7％，55例で，うち89％が3個以下の症例である．

　今回，計5個の多発性脳動脈瘤を有し，うち2個は巨大脳動脈瘤が中大脳動脈に対称性に存在した1例を経験したので報告する．

We report a case of five aneurysms, including three giant aneurysms, a pair of symmetric aneu-rysms, and two fusiform aneurysms.

The initial clinical symptom was a cerebral ischemic attack. On admission, the patient complained of sudden onset of speech disturbance. Physical exami-nation showed left hemiparesis. Radiological studies, including computed tomography and cerebral arterio-graphy, revealed multiple low density areas bilaterally in the basal ganglia and three fusiform giant aneu-rysms bilaterally in the middle cerebral arteries and basilar artery.
