Spinal dural AVMの3症例—その特徴および治療について

Spinal Dural AVM: Report of three cases

I．はじめに

　脊髄動静脈奇形（以下spinal AVM）のうち，動静脈短絡部が硬膜内ではなく，硬膜表面にあり，灌流静脈が脊髄表面を走行するAVMが新たなtypeとして最近注目されるようになってきた^4,6,8,10）．われわれは過去15例のspinal AVMを経験したが，血管撮影上の所見より3例が硬膜AVMともいうべきこのようなタイプと考えられたので，文献的考察を加えて報告する．

A new type of spinal AVM is reported. The chracteristics are as follows : 1) All the patients are male over 50-years-old.

2) Their symptoms are insiduous and progressive ischemic pattern.

3) All the lesions locate at the thoracic and lumbar levels.

4) Selective intercostal angiography shows a fistulous AVM adjacent to the intervertebral foramen fed by spinal chiral branch and drain into tortuous veins on the spinal cord via radiculomedullary vein. No typical hair-pin configuration by radiculo- medullary and spinal cord arteries contributes to the AVM.
