I．はじめに

　急性硬膜下血調重の原因は，頭部外傷によるもの，動脈瘤・動静脈奇形の破裂など基礎疾患に起因するものが大部分を占めている．一方，明らかな外傷はもちろん，基礎疾患の認められない脳表動脈からの出血に起因する急性硬膜下血種が，1971年Talallaらにより急性特発性硬膜下血腫として報告されている．しかし，回様の報告はいまだ散見されるに過ぎない．われわれは急性特発性硬膜下血腫と考えられる1症例を経験したので，これを報告するとともに今までの文献例をまとめ考察を加える．

In this paper a case of acute spontaneous subdural hematoma is reported with a review of fifteen cases in the literature.

A 48-year-old left handed man suffered from severe headache on a motor cycle but he arrived at his brother's offic immediately after. Three hours later he was found unconscious and soon he was brought to our hospital. On admission he was semicomatose with left hemiparesis. There was no evidences of head injury and the x-rays were normal. CT scan disclosed a high density mass in the left temporal extra-axial region with a remarkable shift of midline structures. His bleeding and clotting time were normal.
