頭蓋内および脊髄くも膜下腔へ播種性転移をきたした原発性脊髄グリオームの1例

A Case of The Intracranial and Spinal Dissemination of Primary Spinal Glioma

I．はじめに

　頭蓋内腫瘍が，くも膜下腔へ播種または転移することはよく知られているが，脊髄腫瘍が頭蓋内へ播種性．転移することは稀である．著者らは，今回脊髄腫瘍が経髄液的に脊髄くも膜下腔を経て，脳室系，さらに一部脳実質内へ播種性に転移したと考えられる症例を経験した．本稿では，この症例の症状とCTの経時的変化，および剖検所見を中心に症例の呈示を行い，あわせて若干の文献的考察を行う．

A case of glioblastoma multiforme of spinal cordfollowed by intracranial dissemination was reported.

A 20-year-old man was admitted on Sep. 22, 1981,with motor disturbance of both legs and loss of urinarycontrol. Neurological examination showed spasticparaplegia with bilateral Babinski signs. Abdominaland cremaster reflexes were absent. There was noabnormalities in cranial nerves and upper limbs.Cerebral CT was normal at that time. Myelographyshowed complete block at the level of 8th thoracicvertebra. Spinal angiography revealed abnormalvascularity at the level of Th₇ fed via anterior spinalartery.
