実地臨床における持続頭蓋内圧測定法の諸問題—各種測定法の比較から

Some Practical Problems and Several Methodological Trials in Continuous Intracranial Pressure Monitoring

I．はじめに

　原因の如何を問わず，重症脳不全の管理において，持続的な頭蓋内圧（intracranial pressure，以下ICP）測定に意義のあることは衆目の認めるところである．しかしその方法は，対象疾患によりまた施設間においてもさまざまであり，共通の場における議論を難しくしている．主に脳外傷などの重症脳障害患者を数多く取り扱う当施設での必要性から，種々のICP測定法について比較検討する機会に恵まれたので，その経験をここに総括する．

The constinuous intracranial pressure monitoringhas been widely applied in intensive or critical care, but there are various kinds of methods for its moni-toring at present.

One hundred and forty-one cases mostly with severehead injury were subject to the intracranial pressurerecordings in the critical care ward in Department ofEmergency Medicine, University of Tokyo Hospital,from October, 1980 through May, 1983, and in Neuro-surgical Unit, Showa General Hospital, in April andMay, 1984.

The authors made several methodological trials forthe monitorings in them and compared with oneanother from the aspect of clinical practice.
