術後6年目に頭蓋外（レジン板上）に再発を来たした髄膜腫の1例

Extracalvarial Recurrence of a Convexity Meningioma on the Resin Cranioplasty Plate

I．はじめに

　髄膜腫は，一般的に生物学的には良性腫瘍であり，手術によって完全に摘出できれば再発を免れるはずである．しかし，全摘出されたと考えられる症例からの再発も稀でなく，諸家の報告によれば，組織診断上悪性像を示さないものの完全摘出後も，一般にその5-15％の症例において再発がみられる．

　最近，われわれは，左頭蓋穹窿部で頭蓋内外にわたって発育した髄膜腫剔出手術から約6年間を経過した後，初回手術前と同じ左頭頂部に腫瘤を生じ，検索の結果，頭蓋外のみの腫瘤で，しかも前回手術の際に代用骨として使用したレジン板上の皮下に生じたものであり，組織検査にて髄膜腫の再発と確認した症例を経験したので報告する．

A case is presented on an extracalvarial meningioma which became manifest as a recurrence six years after a complete removal of the left convexity meningioma.

A 74-year-old female was admitted on July 25, 1977 with a complaint of a mass in. the left parietal scalp. Six years prior to admission, she underwent a total removal of a 30g-weighing meningioma in the left convexity. At operation, the bone flap and the dural attachment of the tumor were resected. Later she had acrylic resin methyl methacrylate cranioplasty. At that time histological examination revealed meningothelial meningioma (with no malignant changes).
