Five cases of classical trigeminal neuralgia (TN) and 10 cases of hemifacial spasm (HFS) were treated by microvascular decompression in the supine lateral position. Vascular cross-compression of the trigeminal nerve root entry zone and facial nerve root exit zone at the brain stem was found in 4 patients with TN and 6 patients with HFS. Offending vessels were SCA, AICA, petrosal vein and basilar aneurysm in TN, and AICA, PICA and vertebral artery (megadolichobasilar anomaly) in HFS.
