Coating手術後8年間で巨大に発育し，かつ多くの血管異常を合併した脳動脈瘤の1例

A Case of Postoperative Massive Enlargement of Intracranial Aneurysm associated with Many Vascular Abnormalities

Ⅰ．はじめに

　嚢状動脈瘤に対する一手術法として，動脈瘤柄部結紮（あるいはclipping）がなされているが，それの困難な症例に対してcoatingが行われている．当科において過去10年間に127例の脳動脈瘤の手術を経験し，11例にcoatingを行った．その中の1例が8年後に巨大に発育し，更に内腔およびparent arteryの閉塞と多数の動脈瘤の出現をみたので，その症例を報告し，若干の文献的考察を加える．

A case is presented in which a 44-year-old female had massive enlargement of an internal carotid bifurcation aneurysm, eight years after coating operation, associated with occlusion of the ipsilateral internal carotid artery and development of contralateral multiple aneurysms.

She was first admitted to the Toyama Prefectural Central Hospital in February, 1970, because of subara-chnoid hemorrhage with left hemiparesis. Bilateral carotid angiography only revealed an aneurysm of the right internal carotid bifurcation.
