Ⅰ.緒言
外傷性脳動脈瘤は,外傷と脳動脈瘤発生との因果関係を疑問視する意見もまぬがれず,報告例は稀である.
最近,われわれは髄膜腫の摘出術をうけた症例に,術後の血管撮影ではじめて脳動脈瘤を認め,再開頭によってそれを確認する機会を得た.これは,術中の血管損傷による外傷性脳動脈瘤と考えられるので,ここに報告する.
This 38-year-old housewife had been operated on for intracranial and orbital meningioma four times, the last being on Oct. 30, 1975. During surgery done one year and three months prior to the last operation, a hemorrhage suddenly occured between the right internal carotid artery and the right anterior clinoid process while it was being rongeured. The bleeding point was not identified but the injury was supposed to be at the origin of the right ophthalmic artery. The hemorrhage was controlled by compressing with spongel.
