最近経験した脳肺吸虫症の3例

Cerebral Paragonimiasis : A Report of Three Cases

Ⅰ．はじめに

　肺吸虫症はわが国では西日本を中心に全国にみられる寄生虫症の一つとして古くからよく知られた疾患であるが，最近は疫学的知識の普及と，治療薬の開発によって減少しているものと考えられてきた．しかし過去5年間にわれわれは3例の脳肺吸虫症を経験し，また九州の他の施設からも脳肺吸虫症の摘出手術の報告がなされており，この肺吸虫症の脳内迷人による中枢神経障害は，臨床神経学上まだ忘れてはならぬ疾患といえる．

　われわれの3例は，いずれも痙攣発作を初発症状とし頭蓋X線単純写での頭蓋内石灰化像が特徴的で，手術によりのう腫性肉芽腫を摘出し，うら1例に壊死組織内に成虫をみいだした．症例を報告するとともに考察を加える．

Paragonimiasis is a benign parasitic disease caused by Paragonirnus westermani and endemic in Japan, mostly in the south-western area.

Although the lung is the primary site of infection, ectopic involvement of the brain is not uncommon. Over 300 cases of cerebral paragonimiasis have been reported since the first case of this disease described by Ohtani in Japan in 1887. The patient with paragonimiasis in the lung and the brain were thought to he decreasing because of the introduction of Bithionol treatment (Yokogawa in 1961) and public health instruction.
