Malignant Suprasellar Teratoma in a Girl with Elevated Serum Gonadotropin: Case Report and Some Considerations for Development of Precocious Puberty

Ⅰ．はじめに

　筆者らは最近，8歳女児のトルコ鞍上部悪性奇形腫例を経験した．腫瘍にはcytotrophoblastと思われる細胞が，更にこの部の電顕的検索で分泌穎粒かとも思われる小顆粒の混在が認められ，尿及び血中のgonadotropinも異常高値を示していたが青春早発症（pubertas praecox，以下PPと略す）は伴っていなかった．剖検所見を含め症例を簡単に報告すると共に，この部腫瘍に伴うPPの発現機序，ことに性差について他の2症例と対比し若干の内分泌学的検討を試みる．

Clinical features, endocrinological observation and autopsy findings of a 8-year-old female harboring a suprasellar malignant teratoma were reported and discussions were made briefly with reference to a sexual difference in the development of a precocious puberty associated with tumors in the pineal-parasellar region.

The patient had diabetes insipidus, visual disturbance, and positive Parinoud's sign. She did not show any stigmata of sexual maturity. Endocrine examinations showed that she was a condition of pan-hypopituitalism except high gonadotropin value in urine and blood.
