脳Cytomegalic Inclusion Diseaseを合併した壊死性脳炎の成人例—ウイルス脳内二重感染の可能性について

Necrotizing Encephalitis with Cerebral Cytomegalic Inclusion Disease in an Adult: Possible Double Viral Infection in the Brain

I．はじめに

　cytomegalic inclusion diseaseは，従来新生児の疾患として知られていたが，近年悪性腫瘍，血液疾患，臓器移植後状態に合併した成人罹患例の報告がふえている。

　一方，急性壊死性脳炎あるいは急性封入体脳炎と呼ばれるものは，おもに側頭葉，島葉および眼窩脳に著しい組織壊死，出血を生じ．神経細胞やグリアにCowdryのA型封入体を認めるもので，そのうちの少なからぬ症例は，脳から単純性ヘルペスウイルスが分離されるヘルペス脳炎である^15,37）。

　A Japanese woman of 51 years of age developed high fever and meningeal signs followed by delirium and coma in a week. Coma prolonged with flaccid paralysis of extremities, positive Babinski sign, anisocoria, deviation of the eyes to the right, clenched jaws and generalized convulsions occurred a few times.

　Complement fixation antibody titer against herpes simplex virus was 1:128 on 44th day and fell later, while xanthochromia and erythrocytes were seen in cerebrospinal fluid. EEG on 45 th day showed low voltage theta activity without spikes.
