南九州における神経疫学の実態—筋萎縮性疾患を中心として

Epidemiological aspect of neuromuscular disorders in Southern Kyushu.

はじめに

　南九州は神経疾患の疫学を論ずるうえできわめて重要な位置づけをもつ。一つには地理的条件，もう一つには歴史的条件が，この地域の疫学的特異性に大きな役割を果たしていると考えられるからである。近年，レトロウイルスが関与する新しい疾患概念として確立されたHAM（HAM/TSP：HTLV-I associated myelopathy/tropical spastic paraparesis）の発見も，この地域における莫大なHTLV-Iキャリアーの存在という背景なしには有りえなかったことである。HAM/TSPの存在は広く諸外国でも確かめられており，これらとの比較は人種学的要因やepidemicな要因を検討するうえできわめて重要である。また琉球型筋萎縮症に代表される特異な神経筋疾患の存在も，この地域の地理的，歴史的閉鎖性という特殊性を示すよい見本である。そこで本論文では鹿児島県，沖縄県における筋萎縮性疾患の疫学的現状を報告するとともに，この地域にきわめて特異的であると考えられる神経筋萎縮性疾患にも的を絞って検討を行なった。またHAM/TSPについては，神経筋疾患の疫学的考察がその発見過程に大きく関与しており，疫学的検討がいかに重要であったかを示すよい見本であるので，これについても述べる。

We have been doing house to house survey on neuromuscular disorders for these 17 years in Southern Kyushu area (Kagoshima and Okinawa prefectures). We found out that the composition of these disorders is different compared to the other parts of Japan or the world. Several unique disorders were also found out through the survey. In this paper, we described the result of survey from the view point of disease composition. We also reviewed some of the special disorders which were considered to be restricted in this area. The results obtained in this survey were summerized as follows: 1) High ratio of myotonic dystrophy.
