ペインクリニックにおける最近の進歩

Recent advances in pain clinic

はじめに

　わが国にペインクリニックが設けられたのは東大麻酔科が最初で，それは昭和37年である。以来徐々に大学ならびに一般病院に新設され，現在大学病院のみについて見ると，その90％にペインクリニックが設けられている。理想的なペインクリニックはmultidisciplinary teamの参加による運営であるが，わが国の現状ではなお困難である。わが国では麻酔科医が主にペインクリニックを担当し，神経ブロック療法がその診療の主体をなしている。したがってこの20年間，神経ブロック法の進歩は著しく，外国では見られない神経ブロック法の開発，その適応の拡大から，この領域ではわが国が最も進んでいると考えられる。

　そこで以下わが国のペインクリニックの特徴，神経ブロックに関して得られた知見，さらにペインクリニックの適応となるいくつかの疾患について最近の考え方を述べることにする。

Abstract

The Pain Clinic at Tokyo University was founded in 1962 and after this a number of the formation of "nerve block clinic" or "pain clinic" have appeared one after another in Japan. Cur-rently, about 90 percent of the Universities have pain clinic as a part of the department of anes-thesiology.

Ideally, the most efficient line of running pain clinic is likely to be a multi-disciplinary approach, composed of medical specialists of different disci-plines working and collaborating in a well-coordinated manner.
