痴呆の概念とその疫学

Concept of dementia and its epidemiology

I．はじめに

　交通外傷，中毒性神経疾患あるいは脳血管障害など，脳器質性疾患の慢性期に見られる痴呆は，原因治療が困難なため，必然的にケアに重点が置かれることになり，社会医療のうえで重要な課題となりつつある。ことに高齢人口の増加に伴なって老年期痴呆が急増しつつあり，基礎的研究だけでなく老年医療，さらに医療行政や社会福祉など広い領域から具体的対策を迫られている現状である。

　ところで，痴呆dementiaは，もともとラテン語のdemensからきたもので，分裂あるいは解体した心の意味とされる^10）。つまり，広い精神障害の意味を持つていたが，ここでいう痴呆は，器質性痴呆であって，Kraepelinのいう分裂病性痴呆のように分裂病性感情—意欲障害などのために，二次的に知的能力の発揮できない状態とは区別される。

Abstract

Much concern has been recently focused on the problem of dementia because of the increasing rate of the population of aged people. The author attempts to clarify the concept of dementia and to make the reliable and reasonable diagnostic criteria of dementia.

The diagnostic issues in dementia were discussed as to : 1) the degree of cognitive deterioration of dementia ; 2) the evidence of organic cerebral lesions ; and 3) irreversibility versus reversibility of the clinical pictures. Lastly the epidemiological study of age-related dementia was described.
