悪性脳腫瘍の免疫療法

Immunotherapy for malignant brain tumors

I．はじめに

　悪性脳腫瘍の補助療法として，放射線と化学療法の果たす意義は近年確立されてきたが，これらの治療法と並んで免疫療法の効果についても多くの期待が寄せられている。最近，化学療法の進歩に腫瘍細胞の成長解析理論が貢献した役割はきわめて大きいが，これは培養された腫瘍細胞または手術時得られた腫瘍組織について得られる特殊環境下の情報である。免疫療法の観点は常に生体と腫瘍との相互関係から生まれるものでなければならず，その根本にhomeostasisの概念がある。この相互関係は腫瘍細胞と免疫担当細胞との間にも，また細胞性免疫と液性免疫との間についても存在する。また化学療法を考える上で，腫瘍細胞の成長がたとえ抑制されても，免疫機構が崩壊されてしまっては，治療効果よりも，かえって生体の抵抗力の減退と，ひいては腫瘍による再悪化を招くことになる。これはnitrogen mustardのような初期の制がん剤が，強力な腫瘍抑制効果を持ちながら，臨床的には使う意義がなかった結果をみれば了解できることであろう。

　悪性脳腫瘍患者の免疫機能が低下していることは，他のがん患者と同様に，もはや疑いのない事実として知られており，とくに患者のリンパ球機能が低下していることが明らかになっている。

Summary

The role of immunopotentiators (BCG, Leva-misole and Picibanil) for treating malignant brain tumors was studied. Side effects of these agents and the immunological enhancing effects on the circulatory lymphocyte counts, their blastogenic function and PPD skin reaction were presented. The phase II studies of these agents were summa-rized.

Both intradermal and oral administration of BCG have enhanced PPD skin reaction in about 60% of all the patients, and increased lymphocyte blastogenic ratio from 10.7% (before treatment) upto 21.0%, determined by pulse-cytophotometry.
