I．はじめに

　James Ramsay Huntは，1914年に，いわゆる企図振戦を主徴とする3臨床例をまとめ，これをdyssynergiacerebellaris progressiva（以下D. C. P. と略）と命名した^22）。Huntは，1921年にはD. C. P. にmyoclonusおよびepilepsyを合併する疾患として1剖検例を含む6症例を記載し，dyssynergia cerebellaris myoclonica（以下D. C. M. と略）と命名し，歯状核およびその遠心系の原発性変性萎縮を臨床徴候の責任病巣と考えた^23）。

　1922年には，Hunt自身により，"企図振戦"およびD. C. P. の背景疾患についての修正の報告が発表され^24,25），以来D. C. P. やD. C. M. は小脳変性症の一型として位置づけられたり，myoclonus epilepsyの一型として扱われたり，議論が多い。

　In 1914, James Ramsay Hunt presented the clinical histories of three patients characterized by a chronic progressive tremor in association with dyssynergia, dysmetria, hypotonia, adiadochokinesia and asthenia. He summed up the clinical features as a cerebellar asynergia, and regarded as a definite clinical type of nervous disease. This affection was most characterized by generalized coarse tremors of extremities during muscular activity.
