頭部外傷後遺症に関する臨床的研究—特に症例の統計的観察と急性期治療との相関

A clinical study of post traumatic sequelae of head injury

緒言

　数年来，交通，産業の発達にともなつて頭部外傷患者が急増し，その実態は諸家^1〜4）の示すように，年間50万人の頭部外傷患者が発生し，その約50％が頭部外傷後遺症に悩んでいる。このように頻発する頭部外傷患者の急救処置法および急性期治療は最近著しく進歩し，生命の予後については一応の好成績をみているが，急性期の頭部外傷に起因する後遺症に関してはその発生頻度が高率であるにもかかわらず，病態生理の把握が困難であり，また後遺症状の構成には精神的要素および性格，環境なども関与するので複雑多彩となり，一旦後遺症を残したものでは治療が極めて困難である。

　私は頭部外傷後遺症の急性期病態および急性期治療との相関について詳細な報告に接しないので，教室例について興味ある知見を得たので報告する。

The follow-up study on 608 cases of head injury were performed and the relationship between clinical and pathological findings and therapy at their acute stage and the incidence rate of seque-lae. Following points were revealed, suggesting the importance of active and appropriate treat-ment for head injury at its early stage.

1. Seventy percent of the patients of cerebralconfusion and compression and 50% of simple in-jury and concussion suffered from sequelae.
