多彩な内分泌，皮膚症状を呈したポリニューロパチー患者の免疫グロブリン（液性・細胞性）について

Immunological aspect on a case of polyneuropathy associated with pigmentation of the skin and hormonal disturbances

はじめに

　多発性ニューロパチー，うっ血乳頭，全身皮膚色素沈着，剛毛ならびに内分泌症状に単クローン性免疫グロブリン血症を伴った症例が深瀬ら^1）により報告されて以来同様な症例が次々と発表されて来た。西谷ら^2）の本邦報告15例の総括によれば，1）非定型的な形質細胞腫と網内系刺激症状があり，血清中に微量のM蛋白が証明される。2）蛋白—細胞解離を伴う多発性神経炎，3）全身の色素沈着をはじめとする自律神経，内分泌症状（皮膚硬化，浮腫，剛毛，発汗異常，女性化乳房，陰萎，無月経および糖尿病など）の3主徴を示し，さらにその大部分の症例のM蛋白はλ型であることが判明している。われわれも本症候群と一致する症例を経験し長期間観察し得たのでその概要を述べ，さらに経過中の免疫グロブリンの変動について考案を加えたいと思う。

A 35 year old marriaged woman; she sufferedfrom dysesthesia and motor weakness of upper and lower extremities at the age of 31 year and was admitted in medical clinic, Tohoku University Hospital. Generalized dark brawn pigmentation of the skin, hirsutism, edema in lower legs and amenorrhoea were shown and interpreted as hypogonadotropic hypogonadism.
