Microsurgical dorsal root entry zone tomy (MDT) is effective for medically intractable pain and spasticity. In this report, operative techniques and results of consecutive 18 MDT cases for intractable pain and 5 cases of in-tractable spasticity of various etiology have been reported. From the anatomical study of Ranson in 1914, unmye-linating A δ and C fibers mediating pain sensation are grouped at the proximal portion of the pial ring and flowing into lateral part of root entry zone, and going medial part of Lissauer tract.
