片頭痛の治療

Treatment of migraine.

近年，片頭痛発作治療薬として5-HT_1B/1D受容体作動薬であるトリプタン製剤が登場したことにより，片頭痛の病態の解明が進み，片頭痛の治療が大きく変化しつつある。トリプタン製剤の最大の特徴は頭痛発作発現後に有効で，患者の日常生活障害度を速やかに改善させる点にある。本稿ではスマトリプタンの話題を中心に，従来の発作治療薬との比較，トリプタン製剤の位置づけと問題点，予防薬の適応と使い方につき解説した。

　The pharmacological management of migraine usually centers on two approaches : acute treatment and prophylactic treatment. Recently, 5-HT_1B/1Dreceptor agonist, such as sumatriptan, has become available and been shown to have significant benefits over traditional therapies for the acute treatment of migraine. One of the most important advantages of triptans is that these drugs work during the progressing stage of migraine headache.

　In this paper, the topic of triptans and general principles of prophylactic treatment of migraine are discussed.
