抗ウイルス薬の普及，髄液PCR法の導入により単純ヘルペス脳炎の診断・治療の進歩がみられ，脳幹脳炎，脊髄炎を含めた多様な臨床像が明らかにされてきた。一方では長期入院を余儀なくされる難治例も少なくない。本稿では，診断・治療指針，リアルタイムPCRの有用性，非ヘルペス性急性辺縁系脳炎，他のヘルペスウイルス属神経感染症についても言及した。

A guideline for the diagnosis and therapy of herpes simplex encephalitis was mentioned. With the progression of the development of antiviral drugs or polymerase chain reaction (PCR), various clinical forms of herpes sim-plex virus (HSV) CNS infection, including brainstem encephalitis and myelitis, have been clarified. Meanwhile the disease can sometime become fatal, prolonged, or relapsed, in some patients. Other herpesvirus group CNS in-fections are described.
