てんかん外科における脳機能系評価の意義

Evaluation of brain function in epilepsy surgery: significance of functional brain imaging.

てんかん原性領域および周辺脳には，てんかん性機能異常に基づく脳機能障害が存在する。てんかんの手術においては，この脳機能障害を評価することで，てんかん原性領域の局在を推定するとともに，手術予定領域および周辺脳における脳機能の状態を術前に把握し，手術による損失発生の可能性を正確に予測する必要がある。てんかん外科では，これまで，脳機能を評価するために，アミタールテストを含む神経心理学的検査や，皮質電気刺激による脳機能マッピングなどが行われてきた。最近，これらの方法に加え，PETやfMRIによる脳機能賦活画像が導入され，一次体性感覚野，一次運動野，言語野，一次視覚野などに関しては，すでにこの脳機能画像の臨床的有用性が報告されている。しかし，てんかん脳における高次脳機能障害の機能解剖学的機序に関してはなお未知の領域が残されており，てんかん外科の術前後における神経心理学評価と脳機能画像による検討が今後とも重要と考えられる。

The purpose of presurgical evaluation of brain functions in epileptic brain is two folds 1) detection of lateralized or localized impairment of brain functions, which helps to lateralize and/or localize the epileptogenic region. 2) detection of brain functions in the area of surgery, which helps to estimate and avoid postoperative functional impairment in brain functions.
