Epidemiology of neurological infection in Japan

神経感染症の病因別の疫学の検討は，病因診断が詳細な検索や経時的検討を要するため，困難を伴う。われわれは，1989～1991年における脳炎，髄膜炎，脊髄炎の入院患者の頻度を全国200床以上の病院の内科，神経内科，小児科にアンケート調査を行った。これから得られた推定値は，診断信頼性の高い1年間の期間有病者数の下限値と考えた。脳炎，髄膜炎，脊髄炎全体の推定値（対人口100万）は各々2,200±400（17.7±3.2）例，32,000±16,000（258±129）例，650±50（5.2±0.4）例であった。病因　・病原別では，いずれも病因未確定が各々の全体の51％，73％，36％ともっとも多かった。病原を確認し得たウイルス性がいずれも次に多く，各々31％，18％，36％であった。ウイルス性脳炎のうちもっとも多いのは単純ヘルペス脳炎で，推定有病者数（率）は，433±126（3.5±1.0）例で脳炎全体の約2割，ウイルス脳炎全体の約6割を占めていた。

Estimation of the annual incidence of neurological infections is difficult, because the diagnosis of the neurological infections requires the serial and extensive examinations for their pathogens. In Japan, most of patients with these diseases are admitted to hospitals, especially large hospitals, where diagnosis is most reliable. Therefore, the annual numbers of such inpatients at large hospitals in Japan most likely represented the annual minimum incidence of the these diseases.
