病的笑いの神経学

Neurology of pathological laughter

　笑いは人間の本性である。古くより微笑と通常の笑いは程度の差にすぎないとされているが，両者には質的な違いがあると考えたい。前者は非対称的になりその典型は嘲笑であるが，呼吸筋の運動を伴わない。

　骨格筋は随意と不随意の両神経支配を受け，相補的・拮抗的関係にあるが，その差がもっとも顕著にみられるのが病的笑いである。これには，随意支配の麻痺によるものと不随意支配の亢進によるものとがある。前者の典型は情動失禁であり，後者のそれは笑いてんかんである。Fereによって記載された前駆的馬鹿笑いには両者がある。Lingering smile（モナ・リザ症候群），Wilson病，横笑い，痙笑，Angelman症候群，仮面様顔貌など種々の疾患にみられる特徴的な笑いについても述べた。

Normal and pathological laughters are reviewed and the pathophysiology of the latter is discussed from neurological standpoint. Smile and laughter (in the narrow sense of the word) are different not only in quantity but also in quality. The former does not involve breathing muscles and may be asymmetric. Sneering is extremely asymmetric. Japanese smile is an expression of politeness and diffidence but it is hard to understand by non-Orientals.
