悪性症候群の病態生理

Pathophysiology of neuroleptic malignant syndrome.

　悪性症候群（neuroleptic malignant syndrome：NMS）は抗精神病薬の投与により引き起こされる，筋強剛，発熱，自律神経症状，意識障害などを主症状とする致死性の副作用である。近年の身体管理技術の向上，ダントロレンなどの治療薬の出現によって1980年代前半は25％程度であった死亡率は10％未満に減少しているが，適切な診断，治療を行わなければ未だ危険な疾患である^26）。直接の死因として，心不全，心筋梗塞，不整脈，誤嚥性肺炎，肺梗塞などがあるが，rhabdomyolysisとそれに引き続き発生する腎不全やDIC（disseminated intravascular coagulation syndrome）はその中でももっとも重篤で注意を要する合併症である。一方，抗パーキンソン病薬の中断や抗うつ病薬によりNMS類似の症状を呈することが報告され，NMSの概念が広がり，その定義が曖昧となり混乱を来しているが，狭義のNMSは抗精神病薬によるものに限定するべきであり，病態生理についても区別して議論する必要がある。本稿では，抗精神病薬によるNMSについて，疫学・臨床について触れ，病態生理に関する最近の知見について述べる。

Neuroleptic malignant syndrome (NMS), a potentially lethal adverse effect of neuroleptics, characterized by hyperthermia, severe autonomic dysregulation, muscular rigidity, altered consciousness, and laboratory evidence of rhabdomyolysis, has gained widespread recognition in recent years. It has been also reported that NMS-like symptoms were induced by the administration antidepressants and the withdrawal of antiparkinsonian drugs. Various conceptual pathogenetic models of this disorder have been developed, but its precise mechanism remains unclear.
